How Do I Find Care at Home For a Loved One?

The process of finding care at home for a loved one who can no longer cope without outside help and support can be stressful to say the least. It does not have to be this way though; here at Birchwood Care we’re aware of the complications that can come with finding care at home for a family member and seek to make the process run as smoothly as possible.

In this article we will be exploring the process of sourcing care for someone with the three A’s – Assess, Assemble, Assign.

Assessing the situation

First and foremost, before we can move onto the formalities of signing paperwork, we need to get a good idea of the person requiring care. We do this by gathering as much information at the initial stage such as medical history, emotional and social needs, and your personal concerns – we carry this out over a phone call. The idea is to gain basic yet crucial information such as medical history, emotional and social needs, and your personal concerns. We also make sure we have carers who work in your area – we currently provide our home care services to clients in Tunbridge Wells, Tonbridge, and Edenbridge. After the initial phone call, there will be an opportunity for you and your loved one to meet with a designated senior carer in person. This meeting serves as a chance for both parties to get to know each other on a more personal face-to-face level while allowing us to acquire more information. Examples of this include the clients’ capabilities and routine as well as topics such as their religious beliefs and cultural needs. All this information is vital in enabling us to offer a bespoke home care service to all our clients.

Assembling a bespoke home care plan

Once the assessment has been completed and everyone is happy to move forward, we can begin working together with you and your relative to design a bespoke care plan that gives them the level of care and support they need. Using everything we have learned about your relative and a risk assessment taken at the end of the meeting, we are able to propose a care plan that we think will work best for the client. This will then be passed on to the new potential client to review and recommend any adjustments that might be appropriate. Every client has a completely different care plan, as each person has different needs and therefore care requirements. Our clients always have access to their care plan and are able to make amendments at any time, just as their needs will change over time. Once everyone is happy with the proposed care and support plan, we can get the papers signed and move on to the final step, which is…

Assigning a starting date

Now that all of the formalities are taken care of and everyone is happy, we can choose a starting date. We are able to set the starting date at the time that best suits the client. As we now know exactly what the client requires, we can also begin preparing thoroughly, getting to know the care plan inside and out. Should the situation be serious, we are able to set the starting date within 48 hours!

Hopefully this shows the process of finding care really is not the daunting task it is often perceived to be. We place our clients at the heart of everything we do, catering to each and every one of their needs. We also offer a range of different services ranging from taking care of domestic chores, companionship, and household management all the way through to dignified care.
